If you want to feel like yourself again without quietly closing the door on having kids, the most useful lab numbers may not be your testosterone at all. They may be two pituitary signals sitting one step upstream: LH and FSH.
The two-way radio between your brain and your testes
Testosterone production isn't a solo act. It runs on a feedback loop called the hypothalamic-pituitary-gonadal (HPG) axis. The hypothalamus releases GnRH, which tells the pituitary gland to secrete two hormones: luteinizing hormone (LH) and follicle-stimulating hormone (FSH). LH signals the Leydig cells in the testes to make testosterone; FSH supports the Sertoli cells that drive sperm production [1][2].
When circulating testosterone and estradiol rise, they signal back to the brain to dial GnRH — and therefore LH and FSH — down. That's the "negative feedback" part. It's the same thermostat logic your HVAC uses: sense the output, adjust the input [1].
This is why LH and FSH matter so much for a founder keeping fertility open. They're not just numbers on a page. They're evidence of whether the signal from your brain to your testes is intact — and whether your own testosterone machinery is still being asked to run.

Why 'low normal' with a shrug isn't the whole story
A single total testosterone value on an afternoon panel is a snapshot of the output. It says little about *why* the number is where it is. Two men with identical testosterone can have completely different upstream pictures.
The Endocrine Society recommends diagnosing testosterone deficiency using symptoms plus repeated, standardized morning measurements — not a lone reading [3]. Just as important, LH and FSH help classify the *type* of low testosterone. Low testosterone with low or inappropriately normal LH/FSH points to a pituitary or hypothalamic (secondary) origin. Low testosterone with high LH/FSH points to a testicular (primary) origin [1][3]. That distinction changes the entire conversation — and it's exactly the kind of digging a rushed urgent-care visit skips.
Where enclomiphene fits mechanically
Enclomiphene is a selective estrogen receptor modulator (SERM) — the trans-isomer of clomiphene. At the hypothalamus and pituitary, it blocks estrogen from binding its receptor. Because estrogen is a key part of the "turn it down" feedback signal, blocking it removes a brake. The brain reads lower estrogen activity and responds by increasing GnRH, which raises LH and FSH [4][5].
The practical consequence: an increase in LH and FSH is the body's own instruction to the testes to produce more testosterone endogenously — and, via FSH, to keep supporting spermatogenesis [4][5]. This is the mechanistic reason a provider tracks these two pituitary signals so closely. On a SERM approach, rising LH and FSH are the *point*. They're the marker that the upstream signal is working, not being replaced.

Why LH and FSH read differently on TRT than on a SERM
Here's the fertility distinction that matters most to both of you.
Exogenous testosterone — the standard testosterone shot, gel, or pellet — adds testosterone from outside the body. The HPG thermostat senses that high level and does exactly what it's designed to do: it turns *down* GnRH, and with it LH and FSH. Suppressed LH and FSH mean the testes stop getting the signal to work. Intratesticular testosterone falls and sperm production can decline, sometimes to zero [1][6]. The American Urological Association is explicit that clinicians should counsel patients that exogenous testosterone therapy can impair sperm production and fertility [6].
A SERM like enclomiphene aims to do the opposite. Rather than suppressing the pituitary, it works *through* it — nudging LH and FSH upward so the testes keep doing their own job [4][5]. That's why, on TRT, suppressed LH and FSH are an expected (and fertility-relevant) side effect to watch, while on a SERM, the same two numbers are tracked as evidence the intended upstream signal is engaged.
