Your annual physical was seven minutes and one cholesterol line. If you treat that as a full system readout, you're debugging with half the logs missing. Here's the honest engineering comparison.
The myth, stated plainly
The belief is that a standard annual physical and a longevity panel measure roughly the same thing, so the longevity version is just a repackaged upsell. That's a reasonable prior if you've only ever seen the two-line lipid printout. It's also wrong—not because the physical is bad, but because the two tests are built to answer different questions.
A standard physical is a screening tool. It's designed to catch clearly abnormal findings efficiently across a whole population. A longevity workup is a characterization tool: it's trying to resolve *where you sit on the risk curve* while you still feel fine and the numbers still look "normal." Same patient, different resolution.

What the standard panel usually covers
Most primary-care annual bloodwork includes some version of:
- A basic or comprehensive metabolic panel (glucose, electrolytes, kidney and liver markers).
- A complete blood count (CBC).
- A standard lipid panel: total cholesterol, LDL-C, HDL-C, triglycerides.
- Often fasting glucose and sometimes HbA1c.
That's genuinely useful. But notice what the lipid panel reports: LDL *cholesterol*—the mass of cholesterol carried inside LDL particles. It's a good approximation for most people. The problem is that people can carry a lot of atherogenic particles while their LDL-C reads unremarkable, particularly if triglycerides are elevated or particles are small and dense [1][2].
Glucose and HbA1c have a parallel blind spot: by the time fasting glucose drifts upward, the body has often been compensating with elevated insulin for years. Fasting glucose is a lagging indicator [3].
What a longevity workup adds—and the mechanism behind each
The point of the extra markers isn't "more numbers." Each one closes a specific measurement gap.
ApoB — counting the particles, not the cargo
Every atherogenic lipoprotein (LDL, VLDL, IDL, Lp(a)) carries exactly one apolipoprotein B molecule. So a fasting ApoB measures the *number* of particles that can lodge in an artery wall—not just the cholesterol they happen to be carrying. Major lipid guidance now recognizes ApoB as a more direct measure of atherogenic particle burden, especially when LDL-C and triglycerides disagree [1][2]. For a data person, it's the difference between counting packets and estimating by total payload.
Fasting insulin — reading the compensation before glucose breaks
Insulin resistance develops upstream of high glucose. Fasting insulin (and derived indices) can reveal that the pancreas is working harder to keep glucose in range—information a normal fasting glucose hides [3][4]. It's the early-warning log line, not the crash report.
hs-CRP — the inflammation axis
High-sensitivity C-reactive protein is a marker of low-grade systemic inflammation, and it has been studied as an independent signal in cardiovascular risk assessment—information that lipids alone don't capture [5]. It measures a different pathway than cholesterol entirely.
Hormone and micronutrient status
A standard physical rarely characterizes hormones (like testosterone) or micronutrients (like vitamin D or B12) unless there's a complaint on file. Yet these shape energy, recovery, bone, and metabolic health. Testing them is characterization, not treatment—and abnormal values are interpreted by a clinician, not automatically acted on [6][7].

Why your GP often won't order the broader panel
This is the part that frustrates the skeptical-but-reasonable patient. Primary care operates on screening guidelines and insurance medical-necessity rules: tests are ordered when there's a coded reason. A curious 41-year-old who feels fine often doesn't meet the threshold for, say, fasting insulin or ApoB reimbursement. That's not your doctor being lazy—it's the system optimizing for population screening, not individual characterization. For the quantified-self reader: labs are the missing ground truth
If you already export CGM traces, HRV, and sleep into spreadsheets, you know the limitation: wearables are excellent at *trends* and poor at *absolute biochemical state*. A continuous glucose monitor shows glycemic variability; it doesn't tell you your fasting insulin, your ApoB particle count, or your inflammatory tone. Those require blood. The value of a longevity panel for someone already deep in self-tracking is that it anchors your device data to validated clinical measurements—so a clinician can interpret your recovery plateau or metabolic patterns against ground truth instead of vibes [3][5].
